Boca Rio  and Sand Hills are the only ones that I could think of.

Reserve Vineyards North 5-4-3, 3-4-5.  

Before it was chopped up and its name changed from Spring Hills to Albany Events center every three holes had a 3,4 and 5 in it, you never played the same par two holes in a row, and this was true using any starting hole in a shotgun start.

Before the nines were reversed, Rooster Run, a muni in Petaluma, CA, started 5-3-4 and finished 4-3-5.

The pre-2nd renovation version of Palo Alto Muni had 3 of the 4 possible (start or end for each 9 holes).

Holes 1, 2, 3:  par 5, 4, 3
Holes 7, 8, 9:  par 4, 3, 5
Holes 10, 11, 12: par 4, 3, 5

But a 4, 4, 4 finish on 16, 17 and 18.

4-5-3 start and finish in that order

1 and 16 (interstingly enough the #1 and 2 hcp hole) par 4
2 and 17 only two par 5 on the course
3 and 18 par 3

Honors Course  4-5-3  3-5-4.  Pete Dye should be fertile ground given his common mix of 3,4,5 finishes.  I just don't know enough of his courses.
