I'm almost certain that no bunkers were ever filled in on #11.  The #11 green complex is an example of a site that  just doesn't need them - the hazard on 11 is the Little Lehigh River.

At Lehigh and Lancaster, Flynn makes masterful use of small rivers, leading to wonderful strategy and shot values.  Lehigh #11 is such a hole.

Quick note on the original 1910 Lehigh 9 hole course, which was designed by Whitemarsh Valley pro Jimmy Campbell.

I have a James Campbell doing the original Equinox course in Manchester, VT in 1896.  Not sure if it was the same guy.

I'd have to check my copy of "The Nature Faker" which I don't have with me at present, but if memory serves those grass bunkers on 11 were there by design, not abandonment.   I may be wrong.

I'm not sure if the Whitemarsh Valley James Campbell was the same guy who laid out the original Equinox course, but I'd refer you to this link at Peter Trenham's awesome website where there is a profile of the Philadelphia Campbell that you may find helpful in tracking this down.  You may find some other things there of interest, as well.   Thanks!
