I think Madrid probably got less visits because it was farther away and i do think the contractors got some freedom. Colt was one of the very first to make sure he had dedicated contractors that understood what he wanted and could follow plans, but i think he gave a good contractor some leeway too.
One thing we shouldn't forget is that golf courses were built much more slowly in those days: with man labour rather than bulldozer. So it was much easier to tweak the design as it progressed. You didn't have to plan everything to the nth degree to get your ideas into the ground.
For reference, at Kennemer in Holland, the links took 2-3 years to build and during that time Colt visited about 12 times and made many iterations. Granted Madrid is a lot further away so he perhaps only visited there a few times. He may have revisited in later years when working relatively nearby at Cannes and Pedrena?
