I've seen a ton of really long tee boxes in my time but rarely very wide ones. In the right application it would seem to be a most interesting thing to do on golf holes conducive to it somehow. What are some really wide ones out there that add great interest because of it?

(I may be mistaken but I seem to recall Atlantic City G.C. had a very wide first tee box that sort of melded into the practice putting green or something)

The 17th island green at TPC Sawgrass.  Different angles and different distances.  I think the range is 100 yards to 135.  Tillinghast writes extensively about this for par 3's.  Great idea for spreading wear and tear and creating different angles on par 3's.
The architect would have to work hard to make it work though.

I can't think of examples of one wide tee on par 4 or par 5.  Maybe the first and tenth at Inverness?

I try to find a really wide or laterally split tee in every new design for the reasons you mention.  If we have holes with one tee to two fairways, I think two tees to one fairway would introduce a lot of variety.

Mike Strantz used to do that often on his short par-3 holes.

I've tried it a few times ... the Heathland course at The Legends has a number of them.  The wide, staggered tees on the 9th and 11th holes are used in both directions.

You don't see it too often because it only works on relatively treeless holes ... you're not going to clear extra wide off the tee to build an extra wide tee.

Weiskopf and Morrish designed a boomerang tee box, perhaps 50 yards wide, on a short par four at The Rim. Even more unusual was the fact that the two tee markers were set on opposites ends of the tee when I played there, given the player extreme latitude to tee the ball.   Coupled with  alternate fairways and an extremely wide green it was option overload, in a fun way.

"option overload"!

I like that one. Might have to turn that one into a new catch phrase. Good courses are all about interesting and functional options and when those options are in some form of balance or equilibrium (like Riviera's #10) it can get awful interesting. I think the new name for Riviera's #10 should be "Option Overload."

You can generally tell on the tee box with some players if a hole is presenting them with "option overload". The way you can tell is when they're trying to figure out which side of the tee box to tee off from (this happens all the time with tour pros on on Riv's #10 tee box). They start wandering around on the tee box in smaller and smaller circles like a little dog trying to figure out where to take a dump.
