#12 West Shore Country Club: Camp Hill, PA

+1 on West Shore's 12th

Other good ones fairly close to West Shore:

Hershey CC - West #13 (Beautiful natural terrain for a very good golf hole)
CC York #14 (Crazy downhill sweeping hole that can easily produce anything from 3 to X)
Baltimore CC - Five Farms #14 (Classic Tillinghast Great Hazard hole on a wonderful course)
French Creek #6 (All uphill that requires a lot of good golf to par - green might be a little too severe for how tough the rest is)

Not necessarily my first choice in each spot, because I tried to mix in a few good ones that haven't been mentioned yet.

1) ShoreAcres
2) Camargo
3) Seminole
4) Royal Melb West
5) Cypress Point
6) Carnoustie
7) Portstewart
8) Formby
9) Sommerset Hills
10) The Island
11) Royal Lytham
12) Royal County Down
13) Augusta National
14) Royal St George's
15) Kawana Fuji

My favorite sets of p5's by course are:
