The current island green at the 6th hole at East Lake began life as an island green to a long par 5 designed by Bendelow (probably in 1905) and built in 1906.

The green took its current form when George Adair converted East Lake to its modern routing in 1912.

The hole at Galen Hall mentioned in the NYT article is " The Moat Hole"  and I think it's a different iteration of an "island green." Take a look:





The author makes no reference to Tillie's other "Moat Hole" built in 1925 at The Abington Club  in the Philly 'burbs.

Minute detail but the island green at Shackamaxon is now #18. They changed the routing a few years ago.

Tillinghast's 1929 Old Orchard course (Eatontown, NJ) has an island green on the par-5 7th hole.

For a number of years I believed Old Orchard was Tillinghast based on largely circumstantial evidence. 

I learned a year or so ago that the course was actually designed by the pro at Plainfield at the time, Martin O'Laughlin, who did a number of NJ courses during the late 20s, early 30s, and who is generally credited with the implementation (design?) of the three replacement holes (13, 14, 15) at Plainfield around 1930, as well.
