Acquasanta 1903 (first 9) and 1913 (second 9). I believe anything else built in the timeframe you mention does not exist anymore (e. g. the course built in Sesto Fiorentino, replacing the earlier track in the Parco delle Cascine and later to be replaced by today's Ugolino). The next oldest, still existing course may actually be Villa d'Este from 1926 - I'm not entirely positive on this, but it can't be far off.

1 = Chiberta hole 2 or 11
4 = Toxandria 14
5 = R Hague as Mike said (very very good he saw it!)
9 = Houthalen 6?
11 = Fontainebleau 12
12 = Eindhoven 8 (since changed)
17 = Medoc??
18 = Roosendaelsche 16 (not in GW top 100 list)

All correct, 1=chiberta 2nd, 17=Medoc 9th

Alfonso and Keith are right about no. 15. It is indeed Golf de Paris St Germain (the short 7th).
