A few that I don't think have been mentioned:
The tee shot on 14 at Rustic Canyon (Moorpark, CA) from the black tee marker is impossible for most, especially into the wind. The carry on 7 at Rustic to the right fairway is also difficult, but potentially worth the risk.
The tee shots at 17 and 12 at Manele Bay (Lanai, HI) are both very difficult carries over the ocean, with no real bail-out. (The holes share a tee box and play in opposite directions.)
The approach shot on 13 at Sandpiper (Goleta, CA) is pretty tough for those going for the green in two or laying up short of the ravine and going for the green in 3.
Number 3 at Lost Canyon Sky (Simi Valley, CA) is only 355 yards, But most of it is carry off the tee.
