The 5th tee/4th green at Rockaway Hunting Club may deserve honorable mention on this one.
The line for the tee shot on the par 3 fifth (medium - short iron depending on the breeze) is just to the right of the back of the fourth green. It does take one awful chunked snap hook for the middle of the fourth green to be in the way. However, a pulled tee shot towards the left bunker - which is the bail out shot away from the OB - does cross the back fringe of the fourth green.
However, the honorable mention is deserved because 1) it has happened and, most importantly 2) players on the fourth green ALWAYS stop play and step aside to wait for the fifth tee to clear.
Better safe than sorry (or distracted).
