The Babe at Industry Hills - 18th green is nowhere close to the first tee....across the hotel/practice green/snack shack property and 500 feet of elevation.

And no, that is not the only bad thing I can say about that golf course. ;D

This was the first one I thought of.  It's a much longer cart ride now that they apparently no longer use the funicular tram to get you back up the hill. 

All three of the nines at Moreno Valley Ranch finish reasonably close to where they start (though still kinda far), but none of the nines start or finish near any of the others.  The result is that if you play 18 over two different nines, you will never finish anywhere near where you started.

I'm not sure what the cutoff distance is, but I think The Course at Wente Vineyards (Norman), TPC San Francisco Bay @ Stonebrae, Deer Ridge and Morgan Creek all qualify.
