Delamere belongs in the second rank but worthy of mention.

Marcus, how could you overlook Beau Desert if you recalled Delamere?   ??? ;D

Truly a downhill hole on a great walkable course...

Sean Leary beat me to it - Stonewall may not be Pac Dunes, but it's still darn good.

Wasnt the OP a downhill hole from tee to green ?

Some of these look like a downhill tee shot , to flat fairway second shot ?

Pat,
Yep, hence the "tee-to-green" disclaimer. I've never been there but the second shot looks to be a bit uphill.

James,
Exactly - I started thinking about the question from Pat Mucci's "Doomed" thread.

It's interesting that people have mentioned a lot of nice courses that end with tee-to-green downhill holes, but only SFGC (barely) and Congressional from the Top 50 or even Top 100. If the question were about finishing holes that play uphill tee-to-green, you'd have Augusta, Shinnecock, Cypress Point, NGLA, and Sand Hills just from the Top 10, with Oakland Hills, Riviera, Bethpage, Muirfield Village, and Baltusrol not far behind.

Here's another vote for Wine Valley,

I would also add that Pacific Dunes is a net downhill from tee to green (I think), even though the 2d shot is played uphill.

The other course I would nominate is Saddle Creek in Northern Cali.....a very neat downhill par 5 finisher.

Gullane 1,Colonial. I think Pine Needles qualifies since that is how it is played now.The most underated course in the world in my view.

Yeamans Hall--great downhill par five closer to a classic Raynor layout.

The best courses I can recall with at least a decent downhiller 18th:

Woking (just)
Kington
Beau Desert
Tenby
Southerndown

How about Best Downhill tee-to-green 18th hole?  (regardless of the quality of the rest of the course ...)

Surprised you left Notts off that list Sean although, having not played any of the ones listed except Beau, they could all be crackers.
2014 to date: not actually played anywhere yet!
Still to come: Hollins Hall; Ripon City; Shipley; Perranporth; St Enodoc

Yes, I could easily throw in Notts.  I will tick off Woking because the downhill nature of the hole is really insignificant whereas the final hole at Notts does have the break away approach once the ball is on the green.

Sebonack (Is that downhill enough?), Kapalua, Spanish Peaks, Glenwild, Stonebrae . . .

Would the Camargo Club qualify?  I remember teeing off well above the fairway, but I can't remember if the second shot into the green was downhill.  Either way, it was a strong finish to one of my favorite rounds of golf.

One of the most famous 18th - Kapalua!

Doesn't get more downhill than this hole.
