Blackhawk CC in Madison WI (technically the bedroom community of Shorewood Hills) ends up an uphill, semi-blind par 3.

Kettle Moraine, a fairly obscure 18-holer in the outer reaches of the Milwaukee WI suburbs that's actually pretty good, ends with a downhill par 3 of about 180 yards.

The 18th on #1 course has always been a par-3.  They did have to move it when they reworked #4 course though.

Merrill Hills Country Club - Waukesha WI, Although for the recent Wisconsin State Amateur played as the 9th hole as the committee did not want the tournament to finish on a par 3. (Only me speculating)

Johnson Park Golf Club - Racine WI, A muni with a solid finishing par 3 over the root river which for tournaments can be spectated up on the top left viewing area some 30 feet above the level of the green.

When I was searching around I did find this Bernard Darwin quote that Dan Kelly had posted in 2003 - thought I would share it with you again.

Bernard Darwin, "The Golf Courses of the British Isles," Chapter XIV, "Wales":

" ... [Porthcawl] ends with an exceedingly difficult single-shot hole. There is in the minds of many a prejudice against finishing with a short hole, and it is certainly an ending which is not to be found on many good courses. Nevertheless, if the shot be only difficult enough, it is a little hard to see why a short hole should not make a really fine finish. There is an unpleasant feeling of finality about the tee-shot at any short hole, which never allows us to feel wholly comfortable, and certainly 'Hades' or the 'Maiden' would be infinitely more alarming if they came at the end of the round instead of in the earlier part of the round, when no mistake is irreparable. From the spectator's point of view, it is desirable to get the player to the eighteenth tee in the last state of nervous exhaustion, and a tricky, difficult one-shot hole accomplishes that rather inhuman purpose to perfection."

Sandpiper GC in Santa Barbara ends with a pretty non-descript par 3 of about 175 yds.

Cascades in VA finishes 5-5-3.  Very interesting "flow" at the end and a pretty good par 3 finish.

Remember that 15 is a par 3 as well.  The 3-5-5-3 finish is unique to my knowledge.

Pelham Country Club ends with a par 3 over water. Not much of an architectural interest expect that the NY Athletic Club, the founders of Winged Foot are always eyeing the place. Witnessed a hole-in-one there once, didnt get me a drink!

#1 on the Stoney Lick nine at The Woods Resort in Hedgesville WV is a big par 3!  Not a classic course... but a lot of fun.
