The par-3 sixth hole at Royal Portrush is named "Harry Colt". However, that's the only old-time course I can think of which named a hole after the architect.

The funny tidbit is that this hole isn't really Raynor's design; it was a modification of a runner-up in the Country Life competition from 1914, which Raynor particularly liked.
