Is your source Colt's essays and if so, how is it?
Regarding the second quote, I hadn't thought about it before, but many memorable colt holes play from, if not an elevated tee, then a tee from which the green can be seen. I have in my minds eye a number of par 3s and 4s that play across valleys, hollows, whatnot.
Did colt popularize this - is this one aspect of his being credited as bringing an "active" design intent to the game?
