This one is located West of Chicago.  I post it with the Proviso that I don't know what course it is though I have an idea.

Ryan,

Not Arrowhead.  This course no longer exists.  Dead and buried like the folks in the cemetary immediately to the west sometime in the 1950's.  Does the design work look familiar?

With a little digging I was able to determine the course was Hillside Municipal Golf Course designed by Tom Bendelow.  It was ploughed under to make way for Proviso West H.S. which opened in 1958.  I lived less than two miles from there the first two years of my life and if my father was still here I'm sure he played there and could provide some info on this course.

When I found this I also found another course nearby that also is NLE.  It is built around an old limestone quarry that opened in the 1800's and provided stone for many of the first streets built in the near western suburbs.  Here's is another view of Hillside and the course near the quarry.  Mt Carmel Cemetary is located to the west of Hillside Muni.  The Eisenhower Expressway now cuts between the two courses.

This is another lost Bendelow, Maywood Golf Club (the third site for the club, it appears), dating to 1923. It's at Harrison (south boundary) and Mannheim (east boundary). I believe it closed in the 1960s.
Maywood had opened at another site in the village of Maywood in 1900, then moved elsewhere in 1909 upon losing its lease. That site is unknown to me, but the club eventually moved here.
Mt. Carmel Cemetary, incidentally, is Chicago's most famous mob cemetary. Capone and many "friends" are buried there, along with a few cardinals. Bendelow is not buried there. He's at Forest Home Cemetary, about four miles to the east. But I digress.
